I am the parent or legal guardian of the participant named in this registration. I have the legal authority to consent to their participation in all program activities.

I understand that basketball involves physical activity and inherent risks, including but not limited to sprains, fractures, collisions, and other injuries. I acknowledge these risks and accept them on behalf of my child.

I release and hold harmless Hoopful Union, its staff, coaches, volunteers, and facility partners from any and all claims, damages, or liabilities arising from my child's participation in program activities.

I certify that my child is physically fit and mentally capable of participating in basketball activities. I will notify program staff of any medical conditions, injuries, or limitations that may affect participation.

In the event of an emergency, I authorize program staff to seek medical treatment for my child. I understand I will be contacted as soon as reasonably possible and am responsible for all medical expenses incurred.

I understand the signs and symptoms of concussions and agree that my child will be removed from activity immediately if a head injury is suspected. Return to play will require written medical clearance.

I acknowledge awareness of sudden cardiac arrest warning signs including fainting, dizziness, chest pain, and shortness of breath during exercise. I will inform staff of any relevant family history or cardiac conditions.

My child is expected to demonstrate respect, integrity, and positive sportsmanship at all times. Inappropriate behavior, bullying, or unsportsmanlike conduct may result in removal from the program without refund.

I agree to model positive behavior and respect toward coaches, staff, officials, and other families. I will not interfere with coaching, criticize players, or engage in confrontational behavior.

I am responsible for timely drop-off and pick-up. Supervision is provided only during scheduled program hours. My child must be picked up promptly at the conclusion of each session.

Hoopful Union reserves the right to modify schedules, locations, or programming as needed due to weather, facility availability, or enrollment. Families will be notified of changes as soon as possible.

Registration fees are non-refundable unless the program is canceled by Hoopful Union. Transfers to future sessions may be available on a case-by-case basis with advance notice.

Hoopful Union is not responsible for lost, stolen, or damaged personal items. Please label all belongings and avoid bringing valuables to program activities.

I consent to receive program updates, schedules, and important announcements via email, text, or phone. I am responsible for keeping contact information current.

By completing this registration, I confirm I have read, understand, and agree to all terms and conditions outlined above. I accept full responsibility for my child's participation in Hoopful Union programs.
